Nurse Practitioner or Physician Assistant – Cardiovascular Care (North Chicago Nursing Home Focus) - New Grads Welcome

We are looking for a dedicated Advanced Practice Provider (NP or PA) to join our growing team, delivering compassionate cardiovascular care to patients in nursing homes and post-acute care settings across the North Chicago area.

This position offers an exciting opportunity to make a meaningful difference in the lives of patients with cardiovascular conditions, while working in a flexible, team-oriented environment.

What You'll Do:

• Provide direct cardiovascular care in nursing home and extended care environments

• Work collaboratively with cardiologists, nursing staff, and facility care teams

• Conduct assessments, develop treatment plans, monitor progress, and coordinate follow-up care

• Educate patients and families on condition management and preventive strategies

Position Details:

• Location: Nursing homes and rehab centers in North Chicagoland and surrounding areas

• Schedule: Monday through Friday (day shifts only)

• On-Call: None required

• Employment Type: Full-time, W2

• Compensation: Highly competitive salary + full benefits (health, retirement, PTO, CME, etc.)

Preferred Background:

• Experience in cardiology, geriatrics, or internal medicine is ideal

• Certified and licensed as a Nurse Practitioner or Physician Assistant in Illinois

• A passion for providing high-quality, patient-first care

• Able to work independently and within a multidisciplinary team

• Comfortable with travel to multiple local facilities
