Daytona Heart Group is seeking a Nurse Practitioner or Physician Assistant to join our growing cardiovascular team in a unique Heart Failure and Value-Based Care role. This position offers the opportunity to work closely with experienced cardiology providers while helping manage and improve outcomes for patients with complex cardiovascular conditions.
We are open to both experienced APPs and motivated new graduates who are eager to build a career in cardiology. The practice is committed to providing training, mentorship, and ongoing professional development for the right candidate.
Position Highlights
- Focus on heart failure management and value-based cardiovascular care
- Combination of outpatient and inpatient responsibilities
- Work alongside a collaborative team of cardiologists and advanced practice providers
- Manage follow-up visits and longitudinal patient care
- Participate in hospital rounding and care coordination
- Review and interpret diagnostic testing
- Help identify opportunities to improve patient outcomes and quality metrics
- Support transitions of care and chronic disease management initiatives
- Monday through Friday schedule
- Shared call responsibilities

Qualifications
- Board Certified Nurse Practitioner or Physician Assistant
- Eligible for Florida licensure
- Interest in cardiology, heart failure, or chronic disease management
- Prior cardiology experience is welcomed but not required
- New graduates are encouraged to apply
